Anyone wishing to participate in gaming in the City of Mandan must first make application through the Mandan Police Department. Use the online form to submit any questions to our Gaming officer Sgt. Mike Breid.
To apply for an annual Gaming Employee Permit, print and complete the Gaming Employee Permit application found on this site. There is a $10.00 fee. In the case of lost Gaming Employee ID cards, there is a $5.00 replacement fee.
To apply for a Local Permit or Charity Local Permit, complete the Local Permit / Charity Local Permit application found below. There is a $25.00 fee.
